Atropine sulfate
Indications/Uses
Mydriasis &/or cycloplegia. For cycloplegic refraction, for pupillary dilation desired in inflammatory conditions of the iris & uveal tract.
Dosage/Direction for Use
Refraction Adult 1-2 drops 1 hr before refraction. 1% soln not recommended for use w/ childn. Childn 1-2 drops of ½% soln to each eye bid for 1-3 days. Uveitis 1-2 drops of ½% soln to each eye up to tid.
Contraindications
Hypersensitivity. Primary glaucoma or a tendency toward glaucoma eg, narrow anterior chamber angle. Known or suspected angle-closure glaucoma. Childn <3 yr, w/ Down's syndrome, spastic paralysis or brain damage.
Special Precautions
History of susceptibility to belladonna alkaloids. Psychotic reactions & behavioral disturbances may occur. Patients who may be exposed to elevated environmental temp or are febrile. Protect eyes in bright illumination. May increase IOP. Determine the IOP & an estimation of the depth of the angle of the anterior chamber prior to initiation of therapy. Benzalkonium Cl content may cause eye irritation & discolor soft contact lenses. Avoid contact w/ soft contact lenses. Remove contact lenses prior to application & wait 15 min before insertion. May cause drowsiness, blurred vision & sensitivity to light. May impair ability to drive & use machines. Pregnancy & lactation. Childn (fair skinned w/ blue eyes). Elderly.
Adverse Reactions
Local irritation (eg, follicular conjunctivitis, vascular congestion, edema, exudates, eczematoid dermatitis). Severe reactions manifested by hypotension w/ progressive resp depression. Hypersensitivity; hallucination, confusional state, disorientation; dizziness, headache; eyelid edema, photophobia, blurred vision, prolonged drug effect (mydriasis); tachycardia, bradycardia; intestinal obstruction, abdominal distention, vomiting; erythema, rash; pyrexia.
Drug Interactions
Effects may be enhanced by concomitant use of other drugs having antimuscarinic properties eg, amantadine, some antihistamines, phenothiazine antipsychotics & TCA's.
